Healthcare provider accepting new patients

TRYON—Polk residents have a new option for their healthcare needs with the opening of Hopscotch Health in Tryon.

Hopscotch Health, a Chicago-based healthcare company, opened at Dr. Sandra McCormack’s practice on Lynn Road earlier this year. This is the firm’s sixth office, and there are plans to expand further into Western North Carolina. 

Hopscotch chose Dr. McCormack’s location due to her outstanding reputation in the medical community. Dr. McCormack will continue to practice at the site. 

The medical practice provides primary care tailored to meet the needs of its patients through a value-based care model and a comprehensive care team. 

Hopscotch’s model gives each physician and provider additional resources including community health workers, care management nurses, clinical information specialists and patient relationship managers. These additional resources allow physicians to focus more on their patients.

“Because it is integral that Hopscotch serves the entire community, we believe that the best entrance to a community regarding healthcare is through family practices,” said Hopscotch Director of Marketing, Susan Kimmel-Kelly. “In that vein, we selected opportunities with high-quality and strong-reputation family practices as entrance opportunities into each community we serve.” 

In addition to Dr. McCormack, the office also employs the medical specialties of physician assistant Kevin Panebianco, PA-C. The physicians are supported by two community health workers, a care manager registered nurse, two clinical information specialists, two welcome coordinators and a patient relations and care center manager.
