The survey is available online, which is preferred, but can also be as a hard copy.

Residents are encouraged to complete only one survey per household and are also encouraged to share the survey to get as many opinions as possible.

The survey can be found on the Polk County recreation website, located at polknc.org.

Anyone who needs a hard copy can call the Polk County Parks and Recreation Department at 828-894-8199 or email recreation@polknc.org. People may also stop by the recreation office, located at Stearns Gym in Columbus, at 105 N. Peak St., Columbus between 8:30 a.m.-5 p.m. Monday-Friday.
